My NHS client is looking for a part time project manager to take forward the continued development of the joint intermediate care referral management teams established in South West and South East of the county to reduce avoidable hospital admissions and to facilitate timely hospital discharge.
Effectively the teams co-ordinate referrals into bedded re-ablement (Step Up and Step Down), community re-ablement (NHS and LA) and A & E filter and Admission Avoidance initiatives. The teams consist of therapists, social workers and support staff from two local councils and their local community health provider. The service is commissioned by the 2 local authorities and the 5 CCGs.
The Project Manager will need to have excellent Project Management skills, Prince 2 or similar, and considerable experience in intermediate care in an NHS or LA setting.
This is an immdiate start role. It is a 4 month contract to start with the potential to extend and is for 3 days a week.
